Member Preferences Form

Overview
The Member Preferences form is sent to new members as part of the onboarding sequence. It captures personal preferences (favourite types of golf, dietary requirements, communication preferences, etc.) that allow the club to personalise member communications and service.
When to deploy: Phase 2 — deploy alongside the New Member Onboarding Sequence.

QA Checklist
- Submit a test response using a dummy contact
- Confirm all preference data is captured on the contact record
- Confirm the form link in the onboarding email works correctly
- Confirm a thank-you confirmation appears on submission
